Official Rules

OPEN ONLY TO EYE CARE PROFESSIONALS REGISTERED WITH VISIONWEB.
THIS IS A TRADE SWEEPSTAKES AND IS NOT OPEN TO THE GENERAL PUBLIC.

1. Eligibility: "VISIONWEB’S MONEY MADNESS SWEEPSTAKES” ("Sweepstakes") is open only to eye care professionals ("ECPs") who are 18 years or older, legal residents of the 50 United States or the District of Columbia and are registered VisionWeb members. Employees of  VisionWeb Holdings, LLC (“Sponsor”), or any VisionWeb-connected supplier (defined as labs, wholesalers, distributors, or manufacturers) or buying groups, their subsidiaries, affiliated companies, and agents and the immediate family members (defined as parents, spouse, children, siblings, grandparents) of each such employee, and all those with whom such employees are domiciled, are NOT eligible. Eligibility is subject to the policies of Participants' employer regarding the acceptance of incentive benefits. Failure to obtain Employer's signature on Prize Award Notification will result in revocation of prize. By participating in the Sweepstakes, ECPs warrant that they are eligible to participate under the applicable federal and state law governing procurement of prizes and awards. Sponsors reserve the right to disqualify any ECP if Sponsors determine that ECP’s participation in the Sweepstakes or receipt of a reward or prize would violate state or federal law. Federal anti-kickback law provides for criminal and/or civil liability for inducements related to any product for which payment may be made by a federal healthcare program or federally-funded state healthcare program, including Medicaid and Medicare (collectively, “Federally Funded Programs”). Some states have similar anti-kickback laws (“State Laws”). It is VisionWeb policy to comply with all applicable laws; however, VisionWeb has no way of knowing whether any specific transaction will be paid by a Federally Funded Program or otherwise covered by State Laws. Accordingly, it is an entrant’s sole responsibility to determine if a transaction is eligible for participation in this promotion, and notify VisionWeb of any transactions which should be excluded. ANY TRANSACTION FOR WHICH PAYMENT WILL BE MADE, IN WHOLE OR IN PART, BY ANY FEDERALLY FUNDED PROGRAM, OR WHICH IS OTHERWISE COVERED BY STATE LAWS, IS NOT ELIGIBLE FOR PARTICIPATION IN THS PROMOTION. In consideration for allowing an entrant to participate in this promotional program, entrant acknowledges his/her intent to abide by these terms. VisionWeb disclaims any and all responsibility for entrant’s decision to participate in this promotion. Taxes, if any, are the sole responsibility of the winner. All prize awards are subject to verification by Sponsor. Any fraud with regard to participation in game will result in disqualification of the participant and any prize potentially won will be forfeited.

2. Timing: Enrollment for VisionWeb’s Money Madness Sweepstakes “The Sweepstakes” begins at 12:00 am on January 25, 2012 and ends at 11:59 pm May 5, 2012 (the “Sweepstakes Registration Period”). The time period during which eligible enrollments will qualify for the early registration drawings begins at 12:00 am on January 25, 2012 and ends at 11:59 pm on March 10, 2012 (the “Sweepstakes Early Registration Period”). The time period during which eligible orders (as defined later in these rules) will be measured in order to determine earned sweepstakes entries is March 11, 2012 through May 5, 2012 (the "Sweepstakes Period"). All orders must be sent to supplier by 11:59 pm May 5, 2012 to qualify.

3. How to Play: Each VisionWeb account must enroll in the Sweepstakes to participate in the prize drawings. The Sweepstakes consists of a sweepstakes (based on chance) consisting of early registration drawings, and weekly and grand prize drawings, as follows: Each VisionWeb account (CBU) will receive one entry into the early registration drawings for enrolling in the Sweepstakes during the Sweepstakes Early Registration Period. During the Sweepstakes Period, each VisionWeb account (VisionWeb CBU) will be given a weekly order goal based on previous order history. Weekly orders represent orders placed from Sunday – Saturday. Accounts reaching or exceeding the weekly order goal will earn one (1) entry into the weekly drawings and (1) entry into the grand prize drawing. Accounts ordering to multiple supplier types during the week will receive additional entries into said weekly drawing as follows: order to two (2) supplier types, receive one (1) additional entry, order to three (3) supplier types, receive two (2) additional entries, etc. Accounts ordering to a frame supplier during the week will receive double the total earned entries for the corresponding week’s drawing. Each entry earned during the weekly period of Sunday - Saturday qualifies the account for that week’s drawing. Early registration drawings will take place on Monday March 12, 2012. Weekly drawings will take place each Monday during the Sweepstakes and will consist of entries earned during the previous Sunday – Saturday only. Winners of early registration drawings and weekly drawings are also eligible for grand prize drawings with the same entry. Grand prize drawing will take place on Monday May 7, 2012.

4. Prize Allocation:
Early Registration Prizes (2 at end of Sweepstakes Early Registration Period) – Apple® iPad® 2 mobile digital device - 16GB Wi-Fi ($499 value)

Weekly Prizes (1 per week) – Credit with VisionWeb Supplier of Your Choice ($500 value) and Bonus Prize Selection to be Determined by Weekly Participant Vote ($550 value)

Grand Prize (1 at end of the Sweepstakes Period) – Credit with VisionWeb Supplier of Your Choice ($1,500 value) and AMERICAN EXPRESS® Gift Card ($1,500 value)

5. Prize Winners: Potential winners will be required to sign and return an Affidavit of Eligibility, Liability Release and, where lawful, a Publicity Release, within fourteen (14) days of notification, failing which the potential winner will be disqualified and another winner selected in the place of the disqualified person at random from all entries received. Prizewinner's employer will also be required to sign a Liability Release and Publicity Release. If any prize notification or prize is returned as non-deliverable, the potential winner will be disqualified and another potential winner will be selected in the original winner's place.

6. Other Terms and Conditions: Each ECP participation is subject to verification by Sponsor of entries. BY ACCEPTING THE PRIZE OR A REWARD, THE ECP AGREES THAT THE SPONSOR, ITS SUBSIDIARIES, AFFILIATES, ADVERTISING AND SWEEPSTAKES AGENCIES, AND ALL OF THEIR RESPECTIVE OFFICERS, DIRECTORS, EMPLOYEES, REPRESENTATIVES AND AGENTS, WILL HAVE NO LIABILITY WHATSOEVER FOR, AND WILL BE HELD HARMLESS BY ECP FOR ANY LIABILITY FOR ANY INJURY, LOSS OR DAMAGES OF ANY KIND TO PERSONS, INCLUDING DEATH, AND PROPERTY, DUE IN WHOLE OR IN PART, DIRECTLY OR INDIRECTLY, FROM THE ACCEPTANCE, POSSESSION, USE OR MISUSE OF THE PRIZE OR PARTICIPATION IN THIS SWEEPSTAKES. Subject to all federal, state, and local laws. Void where prohibited.

7. Miscellaneous: If for any reason the Sweepstakes is not capable of running as planned, including due to tampering, unauthorized intervention, fraud, technical failure, human error or any other causes beyond the control of Sponsor that corrupt or affect the administration, security, fairness, integrity, or proper conduct of the Sweepstakes, Sponsor reserves the right in its sole discretion, to disqualify any individual who tampers or otherwise interferes with the entry process, and to cancel, terminate, modify or suspend the Sweepstakes. Sponsor assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to, or alteration of, entries. The Sponsor is not responsible for any problems or technical malfunction of any telephone network or Internet lines that disrupts service to VisionWeb. CAUTION: ANY ATTEMPT BY AN ENTRANT TO DELIBERATELY DAMAGE OR UNDERMINE THE LEGITIMATE OPERATION OF THE SWEEPSTAKES MAY BE A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES FROM ANY SUCH PERSON TO THE FULLEST EXTENT PERMITTED BY LAW.
